一部のマンホールの近くなど、工事作業員の出入りがあるために、有害ガスの放出を行う通風口の近くでは、暗渠化された下水道を流れる下水の臭気が漂うことがある。

^ 主要な水源のひとつであったとされる「将軍池」が病院敷地内に残っており、隣接地が「将軍池公園」として公開されている。この池は大正時代に、もともとの湧水地をもとにして、患者の作業療法も兼ねて池を掘り広げたものだという。

^ 下水は目黒川に並行して走る目黒川幹線を経て、森ヶ崎水再生センターへと送られている。淡島通りやや東に越流枡があり、大雨時には目黒川へも水を流している。そのため大雨のときに越流してくる生下水の水によって水質は悪化し、かつてのように水が褐色に濁り、悪臭を放つ。
