幻肢痛（げんしつう、英: phantom pain）は、怪我や病気によって四肢を切断した患者の多くが体験する、難治性の疼痛[1]。心身症に該当する。 四肢を切断した患者のあるはずもない手や足が痛みだす。例えば足を切断したにもかかわらず、爪先に痛みを感じるといった状態を指す。あるはずのない手の先端があるように感じる、すなわち幻肢の派生症状である。 詳しい原因は判っていない。脳内にある体の各部位に対応するマップが、その部位を失ったにもかかわらず更新されないことが影響しているのではないかという説がある。電気を流した万力で潰されるような痛みがあるという。 痛みを感じているはずの部位は実際には失われているため、痛み止めの薬や麻酔などは効果がない。内部に鏡の仕切りがある箱に失っていない手を入れ、鏡を覗き込みながら（つまり失った四肢の側を鏡で隠しながら存在する四肢を鏡に映して見る）「グー・パー」などと動かすことで痛みが消える、または緩和する、という治療法がある[2]。この療法は鏡療法、鏡治療、ミラーセラピーなどと呼ばれる。ただし、この治療法の効果には個人差があり、決定的な治療法は見つかっていない。 ブレイン・マシン・インタフェースにより制御される義手を使った訓練により、幻肢痛を軽減する研究が行われている[3]。
