周産期死亡率（しゅうさんきしぼうりつ）とは、厚生統計に用いられる用語のひとつであり、年間の1000出産に対する周産期死亡の比率[1]である。
計算式

ここで、周産期死亡は（妊娠満22週以後の死産）+（早期新生児死亡）で定義される。また、出産数は（出生数）+（妊娠満22週以後の死産数）で定義される。したがって、周産期死亡率は、

（年間の周産期死亡率）＝1000×（年間の周産期死亡数）／（年間の出産数）＝1000×｛（年間の妊娠満22週以後の死産数）+（年間の早期新生児死亡数）｝／｛（年間の出生数）+（年間の妊娠満22週以後の死産数）｝

の式で表される。

なお、ICD-10では周産期の定義を、妊娠満22週から出生後満7日未満としている。日本の厚生労働省の統計では平成7年（1995年）からこのICD-10の定義を採用した。

日本において、平成6年以前の周産期死亡率の定義は、（妊娠満28週以後の死産）+（早期新生児死亡）となっていた。
